Terms & services

Our practice is based on fee for service. Fees are due at the time of service. We will provide you with billing receipt with all the necessary codes to submit to your insurance carrier.

We are preferred provider with United Healthcare. We will bill United Healthcare for services.

We accept and process Labor and Industries claims.

We are not a Medicare Provider and have opted out of the Medicare program. This means our services may not be billed to Medicare for reimbursement either by us or our patients.
